You have the right to receive a copy of your health information that we maintain, with some limited exceptions.
Our Health Information Management Department can help you obtain a copy of your medical record. You may request access to your information in writing and also in an electronic format. You also have the right to request that your health information be sent to any person or entity.
To start the process, you may use the Authorization for Disclosure form below.
Mail or fax your completed Authorization for Disclosure Form to the location where you received care. See below for locations.
Copies for patients or their representatives may have a reasonable, cost-based fee. Please contact the Health Information Management Department at the corresponding phone numbers above for further information.

Community's MyHealthMate is an easy, confidential way to access your medical information online and navigate our health system. This free mobile app, which connects to your MyChart account, allows you to see selected portions of your medical records and view your diagnosis, current medications, immunizations and lab results using a computer, tablet or smartphone.
