Community Medical Centers is required by law to maintain the privacy of your health information, provide you with a notice of our legal duties and privacy practices, and follow the information practices described in the Notice of Privacy Practices (available in English and Español.)
You have the right to receive a copy of your health information that we maintain, with some limited exceptions.
Our Health Information Management Department can help you obtain a copy of your medical record. You may request access to your information in writing and also in an electronic format. You also have the right to request that your health information be sent to any person or entity.

Community's MyHealthMate is an easy, confidential way to access your medical information online and navigate our health system. This free mobile app, which connects to your MyChart account, allows you to see selected portions of your medical records and view your diagnosis, current medications, immunizations and lab results using a computer, tablet or smartphone.
